I hereby grant Benson’s Pet Center, its affiliates, representatives, employees, agents, and assigns, the irrevocable
right and permission to photograph, film, record, and otherwise capture my image, likeness, voice, and/or that of my
pet(s).

I authorize Benson’s Pet Center to use, reproduce, publish, distribute, display, edit, alter, and create derivative works
from such photographs, video, or recordings for any lawful purpose, including but not limited to social media platforms,
website content, email marketing, digital and print advertising, promotional materials, in-store displays, and educational
materials.

I understand that these materials may be used worldwide, in perpetuity, and without compensation, and that all images
and recordings are the property of Benson’s Pet Center.

I waive any right to inspect or approve the finished product or the manner in which it is used, and release Benson’s Pet
Center from any claims arising from the use of these materials.

I affirm that I am at least 18 years of age and have the legal authority to grant this release. If the subject is a minor, I
certify that I am the parent or legal guardian and grant consent on their behalf.
